Community Support
The Navigation Program provides personalized support, navigation plans, and emotional guidance for community members experiencing acute or ongoing need. Our program connects community members with vital services, including medical, social, educational, financial, and care coordination. This program supports wellness, autonomy, and improved quality of life through advocacy, education, and strong community partnerships.
Commun’s Navigation Program provides two key services:
Resource Navigation
Real-time referrals and guidance to connect with partner organizations that meet immediate needs.
Clinical Case Navigation
Ongoing assistance with personalized navigation plans, needs assessments, documentation, and emotional support.
History
The case navigation program grew out of the 500 one to one conversations that Commún held in 2020-2021. Case navigation and connections with resources were the most requested community service to meet immediate needs. This program was developed in conversation with community members seeking services, and is staffed by multi-lingual personnel with deep ties in the community.
